Plaquenil Dosage for Malaria Treatment

For adults, the recommended dose for malaria treatment is 800 mg for the first dose, followed by 400 mg for the second dose six to eight hours later, then followed by 400 mg per day for the next two days. Alternatively, a single 800 mg dose may also be effective.
 
For children and infants, the recommended dose is 10 mg per kg of body weight (about 4.5 mg per pound) for the first dose, followed by 5 mg per kg (about 2.3 mg per pound) for the second dose six to eight hours later, then followed by 5 mg per kg (about 2.3 mg per pound) once daily for the next two days.
 
For certain types of malaria, Plaquenil may need to be combined with another anti-malarial drug in order to be effective.
 

Plaquenil Dosing for Lupus

For treating lupus in adults, the recommended starting Plaquenil dosage is 400 mg once or twice a day. After a while, your healthcare provider may recommend a lower dosage (such as 200 mg once daily). The higher the long-term dosage, the more likely that serious Plaquenil side effects (especially eye damage) may occur.
 

Plaquenil Dosing for Rheumatoid Arthritis

The recommended starting Plaquenil dose for rheumatoid arthritis treatment is 400 to 600 mg daily. In some people, the starting dose may need to be temporarily reduced (for a week or so) if bothersome side effects occur. After a good response is obtained (usually four to six weeks), your healthcare provider will recommend that your dose be reduced to 200 to 400 mg daily. The higher the long-term dosage, the more likely that serious side effects (especially eye damage) may occur.
 

General Information on Dosing Plaquenil

Some considerations for people taking Plaquenil include the following:
 
  • Plaquenil comes in tablet form. It is taken once a week to prevent malaria, starting two weeks before anticipated exposure and continuing until four to eight weeks after you return from the high-risk area. For other uses, it is usually taken once or twice a day.
     
  • It is best to take Plaquenil with food or a glass of milk in order to prevent stomach upset.
     
  • Using this medication for malaria prevention in children creates unique problems, since it is available only in adult-strength tablets and since the drug is extremely bitter. However, your healthcare provider and pharmacist can work together to create a solution to these problems for your child.
     
  • For the medication to work properly, it must be taken as prescribed. Plaquenil will not work adequately if you stop taking it too soon.
